As a one-stop healthcare provider, Hougang Family Medicine Clinic @ Ci Yuan Community Club aims to provide quality and comprehensive care for you and your loved ones. Our care team is made up of highly trained medical professionals each with their own expertise. Our Family Physicians, Care Managers, Pharmacists, Medical Technicians, Patient Service Associates and Administrators work hand-in-hand to ensure that you receive quality, holistic and effective care.
Dr Michael Yee is a certified and established Family Physician and Consultant, practicing in his private clinic since 1996. He regularly participates as the principal physician and mission lead in Third World humanitarian medical missions.
Dr Yee obtained the Master of Medicine (MMED) in Family Medicine in 2001, was admitted as Fellow of College of Family Physicians, Singapore (CFPS) in 2005 and obtained the MBA (Healthcare Management) in 2011.
Dr Yee has numerous adjunct teaching appointments with NUS, DUKE-NUS and CFPS. He co-authored the MOH Clinical Practice Guidelines for Osteoporosis and has served on various committees on National Stroke, National Diabetes, Protocol, Primary Care Survey and Transformation of Primary Care Sub-committee. He was also re-elected Council Member of the CFPS.
Dr Victor Teo is an experienced Family Physician in the primary care practice since 2006. He was awarded the Graduate Diploma in Family Medicine in 2012 and accredited with the Family Physicians Accreditation Board in 2013.
Dr Teo has a strong interest in educating new generations of medical professionals, and has an adjunct teaching appointment with Yong Loo Lin School of Medicine, NUS. He believes in treating patients as individuals, and is well versed at building long term doctor-patient relationships.
Our Care Managers are in charge of planning, implementing and evaluating nursing care to ensure that the professional standards are maintained. They also supervise screening for patients with chronic physical or mental health conditions, as well as educate them on managing their own health issues.
Other duties include patient documentation, rendering assistance to doctors, and overseeing collaboration with other departments or community partners to achieve continuity of care.
Our Care Coordinators provide assistance to the Care Manager in their daily duties. This includes conducting basic health assessments such as Body Mass Index (BMI) and blood pressure. They also handle the clinic's administrative work, including patient documentation, laboratory tests and appointments, and provide assistance to doctors wherever necessary.
